Provide the ability to enter a password (and also a username) for secondary participants during a registration

Right now when a participant registers (and the system allows for secondary registrations) there's no way to set a username or password. Username gets set to the email address and I'm not sure if a password is set or not. The only way for the participant to log in is to use the "forgot password" functionality.
