In Surveys, the country field defaults to have "United States" pre-selected and there is no way to have that default changed to be blank or to be another country (say, "Canada"). In Donation Forms, it is possible for admins to set the defaults for country or province, but not in Surveys. The only solution is to try CSS/custom code. This is not ideal for non US-based Blackbaud clients (and their supporters).
