Make survey reports display field labels in column heads, not "question 1", "question 2:"

Surveys are often used for purposes other than a survey. We use these to register people for events when there is no payment involved. (We do not use the event module because we already have a calendaring system on our website.) Our previous form system (Blue State Digital) enabled us to pull a report in a CSV file with column heads such as First Name, Last Name, Name of Synagogue, do you live in the area?, how long have you lived in area?. Pulling a report from Lumninate, these columns would be headed First Name, Last Name, question 1, question 2, question 3, question 4.

Our organization is Jewish Federation of Greater MetroWest NJ, but we are a center or subaccount under the account for Jewish Federations of North America.
