Add field to append URL codes to donation form

We use URL codes to manipulate hidden message fields in donation forms. After donation form published, we add specific codes to the URL manually and use that updated code to update emails, websites, etc. It'd be nice to save those codes somewhere in the DF management, in case someone mistakenly copies directly from the management portal.
