Address, Work Address, Other Address - make them available when we build forms

Donation forms and TeamRaiser Forms. When filling out the form there is no way to ask for address type. The Work Address or Other Address options can not be surfaced so even if they tell us that it is a Company Address we cannot store that information in the proper field. This also impacts the sync to CRM - we have company addresses coming over with the address type of home because we have to map to something.
