For example: Select a fixed donation amount, and then be able to add an additional, donor-entered amount to gift and have it total. I know you can do this with an event registration, but we don't always want this to be the case - donation forms can be used for a variety of things, so it would be good if someone could, say, choose a membership level and add an additional donation onto that.

Hi, this feature is already available under edit Donation Form > Design Donor Screens > Edit donation form > under Giving Patterns & Premiums you can choose Additional Amount field in addition to your Standard Donation Level.
If I've misunderstood the requirement or this doesn't completely solve your need just let me know and I'll reclassify this idea.
