Expiration Date on donation forms should be blank

Expiration date fields should default to blank values instead of defaulting to current month and year, which can be confusing to donors who skip those fields, then receive error message and abandon the form rather than resubmit. Original Bug Case E-67575 was resolved by adding asterisk to expiration date fields but the fields still pre-fill with current month and year.
