Add the email address and phone number of a TeamRaiser participant to the Milestone Admin Notification report

It would be beneficial to receive some basic contact information in the report included in the TeamRaiser Milestone Admin Notification message, so that staff can quickly and easily follow up with new milestones with a personal touch.
