Constituent Email Opt Out report with email name

I would like a Constituent report (constituent on each row) where I can view their Accept Mail status, opt out date, and the name of the email they opted out from. These columns are not options in the custom report writer. I need to look back for any given time frame (year or month) to see the emails that are garnering the most opt outs but by the constituent name and an attribute that I track that allows me to see what their relationship is to my organization.
