Multiple Choice Multiple Response Combo Box Needed

Currently, there is a multiple choice single response combo box option for Convio surveys but there needs to be a multiple choice multiple response combo box option for 'please check all that apply' responses with an 'other' option. A fillable box next to 'other' in this instance would be most helpful.
